She does believe in having a large family, and she is no longer able to have children of her own, so she's made herself believe this is the way to do it. At the same time, I get angry with her for trying to pretend it's all normal and that she's OK with it, because she's totally not. @Erika – I feel so badly for Barb, because I know she does actually love Nikki for having nursed her back to life and has grown to love Margie.
